Duties/Responsibilities:

  • Developrelationshipsandcollaboratewiththeprimarycareteamstoconductsocialservicesassessmentstodeterminetheappropriateneedsforeachmember
  • Plan, coordinate, manage and implement support packages to help members deal with socioeconomic and medical barriers
  • Navigatemanagedcareplansforcommunityservicesandprograms
  • Casemanagementforsocialandbehavioralcaretoallowmemberstoself-managehealthandsocialservicesupport
  • Accurateandtimelydocumentationofpatientencountersandsessionsinallclinicalmanagementsystems
  • Proactivelyidentify methods toimprove ArchWellHealth's approachbased onfeedbackandregularlyconductedsurveys
  • Support advance care transitions with members and their families
  • Assist members with access to state-based prescription programs and other benefits

RequiredSkills/Abilities:

  • Experiencesupportingpatientswithneedforsocialservices
  • Experiencewithscreening,assessment,andplanningforcommonsocialservicesneeds
  • Workingknowledgeofdifferentialdiagnosisofcommonmentalhealthconditions
  • Stronginterpersonalcommunicationskillswithexceptionalactivelisteningabilities
  • Highlyempathetic,non-judgmental,andopen-minded
  • Experienceinacollaborativeteamenvironment

EducationandExperience:

  • FullylicensedLMSWinthedesiredStateofpractice
  • Master'sdegreeinSocialWork
  • 1+years'experienceinclinicalsocialwork
  • Aproblem-solvingorientationandaflexibleandpositiveattitude
  • Missiondrivenandmotivatedtojoinanorganizationthatwilltransformthewaywedeliveraccessible,clinicallyexcellentcaretoseniors
  • ProficientPCskills
  • FluencyinSpanishorotherlanguagesspokenbypeopleinthecommunitiesweserve(wherenecessary)
