ArchWell Health requires all new hires to provide proof that they are fully vaccinated from COVID-19, or represent that they will be fully vaccinated within 30 days of their start date. 

Duties/Responsibilities:

• Developrelationshipsandcollaboratewiththeprimarycareteamstoconductsocialservicesassessmentstodeterminetheappropriateneedsforeachmember
• Plan, coordinate, manage and implement support packages to help members deal with socioeconomic and medical barriers
• Navigatemanagedcareplansforcommunityservicesandprograms
• Casemanagementforsocialandbehavioralcaretoallowmemberstoself-managehealthandsocialservicesupport
• Accurateandtimelydocumentationofpatientencountersandsessionsinallclinicalmanagementsystems
• Proactivelyidentify methods toimprove ArchWellHealth's approachbased onfeedbackandregularlyconductedsurveys
• Support advance care transitions with members and their families
• Assist members with access to state-based prescription programs and other benefits
• Collaboratewiththeprimarycareteamstoconductbehavioralhealthassessmentstodeterminethe appropriate level of care for each member; screen and access each member for commonmental health and substanceusedisorders
• Develop care plans and provide short-term counseling or facilitate referrals, including warm handoffsfortelehealth visitsor referralstoexternalspecialists
• Accurateandtimelydocumentationofpatientencountersandcounselingsessionsinallclinicalmanagementsystems
• Proactivelyidentifymethodstoimprove ArchWellHealth’sapproachbasedonfeedbackandregularlyconductedsurveys

RequiredSkills/Abilities:

• Experiencesupportingpatientswithco-occurringbehavioralhealthandsubstanceabuseissues
• Experiencewithscreening,assessment,andtreatmentplanning forcommonmentalhealthand/orsubstanceuse disorders
• Working knowledge of differential diagnosis of common mental health and/or substance usedisorders
• Highlyskilledinbehavioraltherapy,cognitivebehavioraltherapy,dialecticalbehavioraltherapy,relapsepreventiontherapy,contingencymanagement,andmotivationalinterviewing
• Stronginterpersonalcommunicationskillswithexceptionalactivelisteningabilities
• Highlyempathetic,non-judgmental,andopen-minded
• Experienceinacollaborativeteamenvironment
